If your threads in the head are damaged, you'll have to replace
them. I had one done be Joe Eish, and was very reasonable although I
forget how much. I think Harpers sells the thread inserts only if you
know or are a good machinist.
As for the exhaust nuts, you should drill a hole in it and a fin and
safety wire it in place. If they're totally damaged and out of round
from tightening with channel locks or vise grips or hammers, you should
consider Steve Brenton's nuts, which are works of art. The ones he makes
for headers are pretty good, too.

Do you have bolts? or one big exhaust nut per head?
I have made Exhaust nuts using stainless steel for my Eldo.
