I've also had this problem. The fit of these bowl nuts is really awful.
Even brand new ones. Probably a bad decision about thread pitch for this
application. New washers, even two washers, sometimes helps but I agree
with Bruce's assessment that more frequent checking of these is in
order. I've even seen red RTV on these but don't know I'd do that or
that it would help. Overtightening is a bad idea.
